Queensferry Churches' Care in the Community
Housing Support Service

This service was set up to assist and support older people to remain independent within their own homes and communities. The service will be offered to older people in the areas we cover which are: South Queensferry, Dalmeny, Ratho, Ratho Station, Kirkliston and Newbridge. The service is available to tenants/owner occupiers and is subject to a financial assessment.

If you are in receipt of housing benefit the service may be free.

The service is aimed at older people with housing support needs, which include things like:

• Help with shopping.

• Advice and help with benefit claims and form filling.

• Practical help with domestic tasks.

• Enabling the person to arrange and keep appointments.

• Advocacy and emotional support for the person.

• Arrange assessments by OT for any aids/adaptations.

• Assist/advise on issues around finances e.g. Debt management/ assistance paying bills assistance with collecting benefit/pensions.

• Assistance to become active socially in your community.

Each service user accessing the service will be assigned a named Home Support Worker. An enhanced disclosure (Police Check) is obtained for all Home Support Workers as they are working with vulnerable older adults. All workers will have the necessary qualifications and ongoing training and support to comply with regulations set by the Scottish Social Services Council and the Care Commission.

The service is registered with the Care Commission and will comply with national and local care standards and policies. Copies of inspection reports are available to potential/current service users from QCCC or from the Care Commission office or website.

The Manager of the service will carry out the Initial Assessment and agree a support plan for the person and worker. Ongoing support and regular contact for the person and the worker will be carried out by the Senior Support Worker. The support plan will be agreed upon and updated according to service users needs and/or any concerns by the appointed worker.

This will enable any changes to the support plan to be agreed and monitored to everyone’s satisfaction.
